Administration Department

Issuing Status Cards

Caldwell First Nation will issue status cards to those individuals who have received confirmation of Indian Status from Indigenous Services Canada (ISC) who have not applied for the new SCIS Status card. If you have applied for this card, you must wait for ISC to mail it to you. You must book an appointment with the Administrative Services Manager in advance and bring with you your current status card and 1 photo ID or 2 photo ID to the appointment.

 ​If the card is lost, you will require 2 pieces of photo ID for your appointment. If you do not have photo ID, we can not make a Status card. (NO PHOTOCOPIES ARE ALLOWED AS ID UNDER THE GOVERNMENT REGULATIONS).

If you are reporting a name change, please bring the documents to the office for photocopying so the forms can be faxed to ISC. Drop in appointments are not accepted and we reserve the right to decline the issuance of cards to non-members.

​You can apply for Indian Status by visiting the Indigenous Services Canada (ISC) website or download the following:
  • Application for 16 Years of Age or Older
  • Application for 15 Years of Age or Under
  • Guarantor Declaration
