Membership

Issuing Status Cards

The Caldwell First Nation Membership Clerk will issue laminated status cards to Caldwell First Nation individuals who have received confirmation of Indian Status from Indigenous Services Canada (ISC) and who have not applied for the new SCIS Status card. If you have applied for this card, you must wait for ISC to process it and mail it to you.

You may call ISC for updates to your status card application or Secure status card renewals. Please note that the processing wait times vary
.

You must book an appointment with the Caldwell First Nation Membership Clerk in advance and bring with you your current status card and 1 photo ID or 2 photo ID to the appointment.

Appointments will be made Monday-Wednesday 9:00 am-4:00 pm

Please call to book an appointment with the Membership Clerk - we are unable to ensure the ability to accomodate walkin services.


 ​If you lost your status card, you will require 2 pieces of photo ID for your appointment. If you do not have photo ID, we cannot make a Status card until you have replaced your government ID.
(No PHOTOCOPIES of your ID are allowed under government regulations, you must provide your ID in person). 

Valid ID:
  • Old Status Card
  • Birth Certificate
  • Health Card
  • Driver's Licence
  • Provincial Photo ID Card
  • Firearms Licence​

If you are reporting a name change, please bring the documents to the office for photocopying so the forms can be faxed to ISC. Drop in appointments are not accepted and we reserve the right to decline the issuance of cards to non-members.

​You can apply for Indian Status by visiting the Indigenous Services Canada (ISC) website or download the following:
  • Application for 16 Years of Age or Older
  • Application for 15 Years of Age or Under
  • Guarantor Declaration

If you are requesting status card renewal and have the paper laminated card, please contact the membership clerk, Tammy Jolicoeur via email at [email protected] or by phone at (519) 322-1766 (​x1221).
