Membership

Please contact our office to find out if you are eligible for membership with Caldwell First Nation and ask to speak to Tammy Jolicoeur, Administrative Services Manager and Membership Clerk.

Issuing Status Cards

The Caldwell First Nation Membership Clerk is able to issue laminated status cards to Caldwell First Nation individuals who have received confirmation of Indian Status from Indigenous Services Canada (ISC) - and who have not applied for the new Secure Certified Indian Status (SCIS) card - or are in need of a renewed laminated card, lost their laminated card. If you have already applied, need a renewed SCIS card or lost your SCIS card,  you will need to complete all status card processes through ISC, wait for ISC to complete card processing and you will receive your SCIS card in the mail from ISC directly.

If you would like to have the Caldwell First Nation Membership Clerk assist you with a new laminated status card, you are required to book an appointment in advance and bring with you your current status card and 1 photo ID or 2 photo ID to the appointment. Appointments generally are scheduled between Monday-Wednesday and 8:30 am - 4:00 pm. Please call to book an appointment with the Membership Clerk, Tammy Jolicoeur, directly at 519-322-1766, x1221. 

You may call ISC for updates to your status card application or secure status card renewals. Please note that the processing wait times vary and may take an extended period of time based on ISC application processing times. The Indigenous Services Canada Public Inquiries phone number is 1-800-567-9604.

If you lost your status card, you will require 2 pieces of photo ID for your appointment. If you do not have photo ID, we can not make a Status card until you have replaced your government ID.
(No PHOTOCOPIES of your ID are allowed under government regulations, you must provide your ID in person).

  • Old Status Card
  • Birth Certificate
  • Health Card
  • Driver's License
  • Provincial Photo ID Card
  • Firearms License​

If you are reporting a name change, please bring the documents to the office for photocopying so the forms can be faxed to ISC.

** NOTE: Drop in appointments are not accepted and we reserve the right to decline the issuance of cards to non-members. **

​You can apply for Indian Status by visiting the Indigenous Services Canada (ISC) website or download the following:
  • Application for 16 Years of Age or Older
  • Application for 15 Years of Age or Under
  • Guarantor Declaration

If you are requesting status card renewal and have the paper laminated card, please contact Tammy Jolicoeur.
